It has been nearly six decades since Nepal introduced a family planning program.1,2 At present, the average number of children that woman in is just two, which defined as ‘replacement level’ fertility (that is, two offspring to replace couple themselves).3 In contrast, about fifty-years ago (in mid-1970s), was more than for married Nepal.4,5 This change reproductive behavior (certainly influenced by attitudes towards smaller size) should be considered ‘reproductive revolution’ an essentially patriarchal and patrilocal society like – transition several other countries have experienced recent well.6,7 revolution driven principally adaption modern methods contraception. 1966, became one only handful where officially adopted ‘fundamental human right policy tool long-range national planning’.1 Sterilization (mainly female sterilization) principal method control.8,9 50% women use some form contraception.8 By all measures, country’s program must success. years however, factors including abortion rising age marriage, well male-selected out-migration, also contributed further decline fertility.10-14
